Aim of Dressing

To aid in venous return to reduce the chances of venous leg ulcers re-emerging.

Tips & Tricks

1) Make sure you are comfortable

2) Have the patient point their toes to the ceiling

3) Make sure you do a good heal lock on both the cotton bandage and the Setopress otherwise the dressing will fall apart

4) Ask the patient how they are feeling with the tightness of the bandage. This will save you having to unravel it half way to redo it.

Tutorial Steps

Setup

Make sure you and the patient are sitting comfortably. Have the patient point their toes to the ceiling the best they can

Wrap with cotton bandage

To minimise irritation from the Setopress wrap the leg in cotton bandage in the same technique you will use with the Setopress. It is very important to start below the toes to allow for monitoring of stenosis. Wrap the leg using a heel lock and then 50% overlap up to 2 fingers or 5cm below the back of the knee.

Apply the Setopress (foot area)

Apply the Setopress starting just back from where the cotton bandage started and applying no stretch to the foot area. Apply a heel lock.

Apply the Setopress (above the ankle)

Wrap the leg with a 50% overlap until you reach where the cotton bandage ends. Stretch the bandage as you go up the leg to the correct stretch using the coloured rectangles (in this case green – 20mmHg) to apply the  appropriate stretch. This can be seen when the green rectangles form squares.

Secure

Secure the Setopress using tape.

General Notes

What have we learnt:

1.Make sure your posture is correct. It is best to sit and apply the bandages or have the patient on a bed rather than squatting

2.Always have the patient point their toes to the ceiling to aid the flexion of the ankle and minimise the over tensioning of the bandage around the foot

3.Start the cotton bandage and compression bandage below the 5th toe with minimal to no tension

4.Always remember to do an appropriate heal lock

5.Finish 2 fingers (5cm) below the back of the knee
