It helps if you put sterile gloves on as it allows you to get right into the cavity of the wound rather than using forceps.
Unravel the large kerlix roll and saturate with 5% acetic acid. Don’t wring it out too much as you want there to be plenty of acetic solution on the kerlix.
You MUST leave it for at least 10 minutes. Do not rush this part.
Remove the acetic acid packing and there is NO need to clean the wound again.
Repack the wound with kerlix soaked in iodine. Do not wring it out very much.
You can crepe it however lifting and securing the crepe was an issue with this particular dressing
General Notes
What have we learnt:
1.Also get a big roll of kerlix or lots of gauze to clean the wound
2.You MUST get in side off the crevasses otherwise the bacterial load on the dressing will always be there
3.You MUST wait the 10 – 15min to allow the acetic acid to work
4.Make sure you saturate the kerlix in betadine as it is only once daily
5.Tape it down well (this particular patient had the wound wrapped in cotton bandage and crepe when she got up with the physiotherapists)
