Treatment Goals

Reduce bioburden on the wound bed and remove slough whilst providing a moist wound care environment. The dressing must allow for appropriate footwear to be worn (e.g no cotton bandage and crepe to be used) due to the patient's falls risk

Increase granulation tissue by removing bioburden with a streamlined securing dressing

Step 1

Setup sterile field

Setup your sterile field with all your sterile products

Step 2

Cleanse the wound bed

Cleanse the wound bed by placing the woven gauze (not the pressed gauze found in your dressing pack) onto the wound and saturate with octenilin solution.

Mechanical debridement

Use the gauze covered in octenilin to scrub the wound bed to remove the bioburden and any visible cellular debris or devitalised tissue

Apply primary dressing

Apply the Urgo Clean Ag stick side down over the wound. If it flexes too much you may need to cut slits into it or cut the whole dressing smaller

Apply secondary dressing

If there is noticeable ooze coming from the wound when the previously dressing was removed using a secondary dressing is then required.

Secure dressing

Assess the skin of the patient and if the skin is healthy you can use a sensitive tape like hypafix sensitive but if not secure with a tubular bandage that has been measured to fit the patient's ankle appropriately.
