    Immediately after trauma has occured

    Your patient has sustained a skin tear and it is actively bleeding

     Swipe 
    Step 1

    Clean the wound

    Clean the wound using normal saline and gauze

    Apply Primary dressing

    Apply the alginate dressing

    Step 3

    Secure the dressing

    Using the soft silicone foam dressing apply to affected area. Maintain pressure on the wound bed to help clotting to occur

    Monitor

    Monitor the dressing to make sure there is no active bleeding. Do NOT remove the dressing as this could dislodge the fragile clot that is forming

    Once bleeding has stopped

    Bleeding has stopped and the wound requires a moist wound environment to epithelise

    Clean the wound

    Clean the wound using normal saline and gauze being gentile not to agitate any skin around the periwouund or clots.

    Apply Primary dressing

    Apply a double layer inadine

    Secure the dressing

    Using the soft silicone foam dressing apply to affected area.

    Monitor

    Recheck in 2-3 days
