To remove unnecessary dressings and provide wound care required on only the wounds that require it.

Multiple Small Skin Tears

Patient had multiple skin tears however on review (last dated mepilex border was dated 3 days prior). Patient had 5 - 6 skin tears but some had scabbed over or healed with only one on the patient's hand and his foot that needed dressings.

Step 1

Remove all unnecessary dressings

Remove the mepilex borders slowly and assess each skin tear indiviudually. SOAK any steri-strips or other dressings off

Clean wounds

Clean all wounds

Dress wounds

dress with inadine and mepilex border

Moisturise all other areas

Apply moisturiser multiple times per day
