Treatment Goals

To support clot formation and to have haemostasias return and then wound healing to begin.

Aim of Dressing

To apply atraumatic wound care dressings with NO tape. The dressings will support clot formation but will not cause further bleeding or skin tears

Not actively bleeding

When the roof of the haematoma has been removed in surgery and has been cleaned and there is NO active bleeding

Step 1

Setup sterile field

Setup your sterile field with all your sterile products

Clean the wound

Clean the wound gently as to not agitate the wound bed

Apply alginate

Apply the alginate dressing. There is not right or wrong way to put it down

Cover

Cover the wound with zetuvit (DO NOT anchor with tape). Have the patient or a fellow healthcare worker hold it for you

Secure

Secure the dressing using the appropriately sized tubifast or tubular form from the toes to the knee. One piece covering from the toes to just below the calf and then another piece from the ankle to just below knee

Actively Bleeding

When the above dressing plan does not stop the bleeding and strike through can be seen this wound care plan can be utilised. *** A pharmacist/medical officer should be consulted about the appropriate strength of tranexamic acid ***t

Clean the wound

Clean the wound gently with the extra loosely woven gauze squares. Try not agitate the wound bed. **The gauze in the dressing pack will be used in the next step**

Create Tranexamic acid mixture

Soak the pressed gauze (the one from the packet) in the tranexamic acid solution. Placed directly on do not squeeze out.

Cover

Cover the wound with the biatain and then the zetuvit (DO NOT anchor with tape). Have the patient or a fellow healthcare worker hold it for you

Secure

Secure the dressing using the appropriately sized tubifast or tubular form from the toes to the knee. One piece covering from the toes to just below the calf and then another piece from the ankle to just below knee
