Treatment Goals

Removal of tophi from the wound to allow for secondary intention

Aim of Dressing

To flush and debride the wound of tophi to allow for secondary intention. Secondary intention is sort due to the acknowledgement that the wound edges are non-viable.

To wash out the cavity left after the primary wash out in theatre and pack

Step 1

Wash and clean the foot

Wash and clean the foot well

Clean the wound

Clean the wound with normal saline

Step 3

Slush the wound cavity out

Using a syringe flush out the wound cavity with pure iodine. Look for any communication with the skin and if none then milk out the remainder of the iodine.

Iodine (100ml) (AKA Betadine)

Step 4

Pack the wound cavity

Using a wound probe probe the wound and take measurements. Then CUT the aquacel ag ribbon in half WIDTH ways to allow for easy insertion. Using the probe feed the aquacel ag ribbon into the wound bed. Make sure you get it to the veery bottom of the wound. If you do not this will allow a collection and microbial build up to form.
