Treatment Goals

To remove any cellular debris or puss from the cavity where there is a skin bridge or sinus

Aim of Dressing

Saturate sinus cavities with iodine to remove and/or kill the bacterial load causing the infection

Everyday

Post-operatively after washout

Following days after surgical washout has occurred where the likihood of cellular debris and puss is still in the track ward based iodine flushed ,ay occur before a more long term plan is enacted.

Step 1

Prepare your sterile field

Prepare you sterile field and open up all your sterile wound care products using ANTT

Clean the wound

Using normal saline and gauze clean the wound bed

Flush the sinus with iodine (part a)

You may wish to put on PPE in case their is some inadvertently splashing during the irrigation. Draw up the iodine into the 10ml syringe

Flush the sinus with iodine (part b)

Connect the 10ml syringe to the 18g cannula catheter (with the inducer removed and safely discarded)

Flush the sinus with iodine (part c)

Stick the catheter down the sinus and gently push downw on the syringe to flush the cavity. You may need to augment the wound to help open up the passage. Always look at the distal end of the passage to watch for the iodine coming out

Pack wound(s) with iodine

soaked and lightly squeeze out the betadine soaked gauze and lay into the wound bed(s)

Secure

Cover wounds with zetuvit and cotton bandage and crepe

Ongoing management

Once the immediate infect has passed a more long term management of the wound can begin
